Please visit the Applications page to select the appropriate form. Depending on your browser settings, you may fill it out and submit online, or download and print for mailing to BOSF.
BY MAIL:
If you have had a fire within the past three (3) months and are having trouble with the online forms, we will be happy to mail an application to you. Send us an email at bosfundsa@gmail.com, or send a request by US Postal Service at the address listed to the right.
NOTICE: Be sure to provide a valid mailing address and phone number. The fire loss address may no longer be a valid mailing address.
